MEDICAL INSURANCE AND PAYMENTS
Should you be paying for your treatment with your medical insurance provider we will invoice them
for the cost of your treatment at the end of your sessions, however, you are still liable for the
payment of your fees in full. To enable us to do this we will need the name of your insurer, your
membership number and the authorisation code for the proposed treatment.
If you have an excess to pay on your policy you will be expected to cover that short fall until the
policy kicks-in. If your allowance has been exhausted or our claim has been rejected for whatever
reason, any fees will be invoiced directly to you.

DIRECT PAYMENTS
You will be required to pay for each treatment in full after each session using either cash,
credit/debit card on the premises.

CANCELLATION POLICY
The onus will be on yourself to remember your appointments. To cancel or reschedule an
appointment without incurring a charge, we require at least 24 hours notice. Appointments being
cancelled or rescheduled within 24 hours of the appointment time will be liable to a late cancellation
charge. The charge will be the full fee of the appointment.

CONSENT TO TREATMENT
We will provide you with a copy of our GDPR form which you will peruse and sign giving your
consent for us to share your information only with medical professionals to facilitate your
treatment.
You will be consenting to the physiotherapist performing the assessment in order to ascertain
what is causing your symptoms, the physiotherapist will explain to you the issues and what can be
done, you and the physiotherapist will then decide the best treatment plan. We welcome any
queries on anything that you may be uncertain of and you can stop the process at any time.
